Information No. 5 of the Vice-Rector for Student Affairs of Lodz University of Technology of 27 March 2020

Following the decision to suspend classroom teaching at Lodz University of Technology and pursuant to the Regulation of the Minister of Health of 24 March 2020 amending the Regulation on the declaration of the state of epidemic in the Republic of Poland which introduced restrictions on the movement of persons, effective 27 March 2020 until further notice, I introduce the following changes in the application procedure for financial assistance by students and PhD students:

1. Students/PhD students applying for bursaries, hardship grants, and allowances for students with disabilities shall submit their respective applications only by:

a) sending legible scans or photographs of the original paper copy of a filled out and signed application along with scans or photographs of all required documents (certificates, declarations, etc.) attached to an e-mail message sent from an individual e-mail account in the TUL electronic mail system to res@adm.p.lodz.pl or rss@adm.p.lodz.pl, followed by

posting with Poczta Polska without undue delay a letter with the originals of the said documents addressed to: Sekcja Obsługi Świadczeń PŁ, ul. Żeromskiego 116, 90-924 Łódź

or

b) sending to the ePUAP electronic inbox of Lodz University of Technology: /PolitLodz/SkrytkaESP
a file (or files) with scans or photos of the original filled out and signed application and scans or photos of all required documents (certificates, declarations, etc.) signed with a qualified electronic signature or a Trusted Profile 

or

c) signing the file (or each of the files) containing  a scan or a photo of the original filled out and signed application and scans or photos of all required documents (certificates, representations, etc.) with a Trusted Profile using the tool available at: https://moj.gov.pl/uslugi/signer/upload?xFormsAppName=SIGNER

and next, sending thus generated XML files attached to an e-mail message from an individual email account in the TUL electronic mail system to res@adm.p.lodz.pl or rss@adm.p.lodz.pl

 

2. Students/PhD students applying for the Rector's scholarship shall submit their respective applications between March 14th and April 24th, 2020 only by:

a) sending legible scans or photographs of the original paper copy of a filled out, and signed application along with scans or photographs of all required documents (certificates, declarations, etc.) attached to an e-mail message from an individual e-mail account in the TUL electronic mail system to res@adm.p.lodz.pl or rss@adm.p.lodz.pl, followed by

posting with Poczta Polska without undue delay a letter with the originals of the said documents addressed to: Sekcja Obsługi Świadczeń PŁ, ul. Żeromskiego 116, 90-924 Łódź

or

b) sending to the ePUAP electronic inbox of Lodz University of Technology: /PolitLodz/SkrytkaESP
a file (or files) with scans or photos of the original filled out and signed application and scans or photos of all required documents (certificates, declarations, etc.) signed with a qualified electronic signature or a Trusted Profile 

or

c) signing the file (or each of the files) containing  a scan or a photo of the original filled out and signed application and scans or photos of all required documents (certificates, representations, etc.) with a Trusted Profile using the tool available at: https://moj.gov.pl/uslugi/signer/upload?xFormsAppName=SIGNER

and next, sending thus generated XML files attached to an e-mail message from an individual email account in the TUL electronic mail system to res@adm.p.lodz.pl or rss@adm.p.lodz.pl

 

3. Students/PhD students seeking to resolve a health insurance issue shall submit appropriate forms only by:

a) sending legible scans or photographs of the original paper copy of a filled out and signed ZUS form and declaration attached to an e-mail message from an individual e-mail account in the TUL electronic mail system to res@adm.p.lodz.pl or rss@adm.p.lodz.pl

or

b) posting with Poczta Polska a letter with the originals of the said documents addressed to: Sekcja Obsługi Świadczeń PŁ, ul. Żeromskiego 116, 90-924 Łódź.

 

In specific situations, a student/PhD student may be requested by e-mail or by telephone to appear in person at the Sekcja Obsługi Świadczeń PŁ ul. Żeromskiego 116 in Łódź, Building A-33, Entrance E, 4th floor, in order to provide or explain details of the documentation.
