Ladies and gentlemen,
in connection with the press release of February 11, 2021 regarding the possibility of including academic teachers and other people conducting classes at universities in the COVID-19 vaccination program, below I present you the most important information (confirmed on the afternoon of February 12, 2021) in this regard:
1. The vaccination program covers academic teachers and all teachers with a date of birth after December 31, 1956.
2. An academic teacher who works in more than one university or cooperates with more than one university may apply for vaccination only in one university. The university that is the primary place of work is recommended.
3. Anyone who has already completed the form on the Wikamp platform at: https://adm.edu.p.lodz.pl/mod/questionnaire/view.php?id=5890 and expresses their willingness to participate in the vaccination program does not have to take any action, just wait patiently for the next information, which we will send as soon as we receive it from the Ministry of Education and Science.
4. Anyone who has completed the form on the Wikamp platform, but is not willing to be vaccinated, is asked to send such information at https://edu.p.lodz.pl/zglosproblem by Monday (February 15) by 2 p.m.
5. Each person who has not yet completed the form, but is willing to vaccinate, is asked to register on the Wikamp platform at: https://adm.edu.p.lodz.pl/mod/questionnaire/view.php?id=5890 as soon as possible - the order of messages will be reflected in the order of vaccinations, but no later than February 18, 2021, 12:00.
6. After the National Health Fund designates the place where vaccinations will take place, you will be sent an appropriate message.
7. People born before December 31, 1956 and under the age of 70 will be able to get vaccinated in their age groups. We will publish an announcement on this matter as soon as we receive confirmed information.
8. Currently, efforts are being made to ensure that the current enrollment process and, consequently, the vaccination process also apply to employees from other groups employed at the university. We will also communicate information on this matter as soon as we receive it from the Ministry of Education and Science.
9. We will provide you with any other, more detailed information after obtaining it from the Ministry of Education and Science and the National Health Fund.
