Ladies and gentlemen,
The government of the Republic of Poland updates information on vaccinations on an ongoing basis. Therefore, in order not to send messages after the launch of subsequent years, I provide general information on the currently applicable rules for registering you for vaccinations.
1. In accordance with the constantly updated schedule, staff employed in the Rector's Office send the data of employees who completed the form on the Wikamp website to the Laser Diagnostics and Therapy Centre.
2. Once the Laser Diagnostics and Therapy Centre has received your data and the NFZ has unlocked the vaccination records for each year group, it will book your vaccination appointments.
3. You receive information about the date by means of an SMS sent by the Laser Diagnostics and Therapy Centre.
4. In connection with the above, all persons interested in vaccination at Laser Diagnostics and Therapy Centre who have not yet completed the form are asked to fill in the form urgently at https://adm.edu.p.lodz.pl/mod/questionnaire/view.php?id=5890
5. Employees who have completed the form in the past are asked to wait patiently for the appointment according to the national schedule and, if possible, by the Laser Diagnostics and Therapy Centre.
6. If any of you provided a declaration of willingness to vaccinate at Laser Diagnostics and Therapy Centre in a paper form, it will also be recorded by the employees of the Centre.
