Ladies and gentlemen,
I would like to kindly inform you that, based on the information contained on the website https://www.gov.pl/web/szczepimysie/wkraczamy-w-kolejne-etap-szczepien-…, we are continuing the registration process for vaccinations for people born in the years 1952-1956.
1. The current provisions apply to people born in the years 1952-1956 and not reported to the National Health Fund among academic teachers.
2. After completing the form available at: https://adm.edu.p.lodz.pl/mod/questionnaire/view.php?id=5890, it is possible to sign up for vaccination at the Laser Diagnostics and Therapy Centre of Lodz University of Technology.
3. There are also other methods described in the announcement on the government website.
4. If someone has already completed the above form, they do not have to fill it in again.
5. Vaccinations will be carried out in accordance with the schedule set by the Government of the Republic of Poland (first for 69-year-olds), but not earlier than March 22, 2021. The date will be determined by the availability of vaccines and is not dependent on Laser Diagnostics and Therapy Centre.
6. There is currently no information on immunization for people born after 1956.
7. In the event of a change in the rules or an extension of the groups of people to be vaccinated, we will inform you in a separate message.
